The patient wanted to replace an old case covering the upper right second premolar with a new one. After the clinical and radiographic examination, it was found that the premolar was denervated and had been restored in the past with a cast shaft. According to the treatment plan, it was decided to remove the metal shaft, repeat denervation and make a new all-ceramic case. Because after the removal of all the prosthetic work, it appeared that the tooth was missing all its dental molar and only its root was left, it was decided to make 2 cases (in combination with the adjacent molar), so that the tooth would have better support and the new prosthetic work would have greater resistance to chewing forces and thus a longer duration over time.
